Duodenal Switch/SADI-S (Single anastomosis duodenal bypass with sleeve gastrectomy)

The Biliopancreatic Diversion with Duodenal Switch – abbreviated as BPD/DS – is a procedure with two components. First, a smaller, tubular stomach pouch is created by removing a portion of the stomach, very similar to the sleeve gastrectomy. Next, a large portion of the small intestine is bypassed.

The procedure

The duodenum, or the first portion of the small intestine, is divided just past the outlet of the stomach. A segment of the distal (last portion) small intestine is then brought up and connected to the outlet of the newly created stomach, so that when the patient eats, the food goes through a newly created tubular stomach pouch and empties directly into the last segment of the small intestine. Roughly three-fourths of the small intestine is bypassed by the food stream.

The bypassed small intestine, which carries the bile and pancreatic enzymes that are necessary for the breakdown and absorption of protein and fat, is reconnected to the last portion of the small intestine so that they can eventually mix with the food stream. Similar to the other surgeries described above, the BPD/DS initially helps to reduce the amount of food that is consumed; however, over time this effect lessens and patients are able to eventually consume near “normal” amounts of food. Unlike the other procedures, there is a significant amount of small bowel that is bypassed by the food stream.

Additionally, the food does not mix with the bile and pancreatic enzymes until very far down the small intestine. This results in a significant decrease in the absorption of calories and nutrients (particularly protein and fat) as well as nutrients and vitamins dependent on fat for absorption (fat soluble vitamins and nutrients). Lastly, the BPD/DS, similar to the gastric bypass and sleeve gastrectomy, affects guts hormones in a manner that impacts hunger and satiety as well as blood sugar control. The BPD/DS is considered to be the most effective surgery for the treatment of diabetes among those that are described here.

    It was a scary step but well overweight with heart problems and borderline diabetes it was something i really wanted to do for my wellbeing and its been a fantastic journey that i approached with a positive attitude one i will never forget from my first consultation with the amazing Dr Atolla who was very kind and wise to the great support from Bec on dietary needs and the cheery chats and support from Mary-ann the lovely admin lady that keeps it all moving smoothly pre-op and post -op support from Bec that was simple and not complicated , the surgery , recovery then post-op i wont say it was all a breeze but with the support from the team i kept motoring on looking toward the outcome and YES !!! Febuary was my surgery ive lost around 25kg my heart meds have been reduced my sugars are awesome i feel 10yrs younger and i must say 😊 look 10yrs younger too the best decision ever thank you so much to the team your Awesome !!!!     In 2019, I had an accident where, the short version is that I have ended up with Osteoarthritis in my right ankle & sub-Talar joints & needed fusion. Pain levels were 5-6/10, chronic and worsening. PTSD & brain-fog affected my well-being, ability to read and work. It was a struggle to walk 400m. Two weeks post gastric sleeve surgery my pain levels were down to 2-3/10 and often negligible & while it was about 3-400m too far, I was able to walk from my favorite café to home (1.8km) with no debilitating pain or consequences. Three weeks post surgery, I had read a novel from cover to cover and started back at work. Lo and behold! I am now almost seven weeks post surgery, I am working productively three days a week (like I used to before the accident), I am looking to spend time with my friends, my PTSD testing results have improved, I am driving locally again and I am walking almost daily around my block (1.2km) with no debilitating pain or consequences. I never dreamt that my world would or could turn around like this so soon. I cannot thank Mo, Maryanne, Haley (EP), Becky (Dietician) & Rod (Psychologist) enough for playing such a pivotal role in helping me to regain my sense of who I am. This holistic approach to major surgery has been an incredible experience & I feel blest that I have had such an incredible, professional & caring team to work with, who enable me to recapture the person in me I thought I had lost. While I have lost significant weight, for me this journey has meant so much more and I look forward to seeing the overall weight-loss progress over the coming year. Now two years down the track and I have finally stabilised and added 5kg. But overall I lost 45kg with the surgery and am extremely happy with the outcome. I cannot thank Mo enough for working with me. Through this surgery, I have improved my health and mobility so much much that when I underwent hindfoot fusion of the subtalar and ankle joints in January 2023, my recovery was near perfect and this week, I was finally able to walk around the lake with a single crutch (2km) during rehab and with no pain repercussions. Last week, I even walked 800+ metres without the crutch - my EP held on to it though. I am so grateful to work with a surgeon and his team who take a wholistic approach and are more focused on a person's overall health and well-being. So incredibly grateful.

    When starting this journey I set on my decision and had all my duck in order for it to happen but the nervous and all the other emotions where all consuming until I met Mo, Bec and Marianne . These will be your three biggest supporters. This team truly cares, I had seen other surgeons but never truly aligned goals, expectations and personalities with so find who you are comfortable with. I started out at 118 kg pre op and 6 months post op my weight is 82kg. 36kg down and 16kg away from my goal. My whole self has changed no more anxiety when I leave the house, constantly worried about the people around me looking and laughing. I an no longer constantly hyper aware of my body and how I look etc. I’m now peaceful, happy, comfortable in my own skin and daily movement is no longer a struggle. I would do this process all over again to get my life back and I would go straight to better life surgery team. You’re amazing and look forward to future appointments.
